Welcome to the 2024 Uniform Order Form!

This online form will allow you to complete player information, choose sizes for uniform items, and submit payment. Please read through each page carefully to ensure that all information is provided accurately. Once items and sizes are submitted, there is no way to change sizing.

Payment Specifics

All uniform orders must be completed and paid for through our online registration system. Please be advised that any uniform items will not be ordered until payment has been received. Once the online registration process has been completed and payment rendered, your child’s uniform order is secured and ordered from Nike. If form and payment are completed past the deadline, your uniform order will be processed but will be delayed in receiving. Online payments may be made with a credit or debit card through our online registration system. Following the completion of this registration, you will see a charge on your account from Atlanta Extreme Volleyball Club.
