AEVC 2023-2024 Conflict Form
Please submit all information for each day that the athlete/coach will be missing from an AEVC event. Once submitted, this form will be sent for approval by the director and filed on your account. You must also notify the head coach and assistant coach as well for any conflicts via GroupMe.
NOTE: If your conflict will affect more than 1 consecutive day, you must fill out a form for each individual day of conflict.
